Each resident will be assigned a key worker who will support them with a tailored support plan, including realistic goals to develop key skills and promote independence.
The support plan includes:
Personal Care
Built around the service users’ individual care needs. This may include support with personal hygiene, oral hygiene, using the toilet, and keeping their room neat and tidy.
Activity Planning (Social Inclusion)
We will develop a risk assessment with each service user, and they will be supported in planning their daily activities, such as taking exercise, keeping in touch with family, or making new friends.
Meal Preparation
Service users are supported in planning their daily or weekly meals. This includes making a menu, shopping list, using public transport, and preparing their meals. This can start with something simple until they develop the necessary skills with limited reliance on staff.
Support with Education and Employment
This could include guidance in CV writing, work placements, and/or college placements.
Money Management
Service users will be supported by staff in areas such as budgeting for their shopping, activities, bills, and overall money management.
